About the Walmart/Sam's Club Medical Plan Option
We’re excited to introduce a whole new way of looking at health care. It’s called an Accountable Care Plan (ACP), and it’s designed to improve the quality of your care, provide a better experience for you and your family – and do it all while saving you money. The idea is to connect you with the right provider for the right care at the right time.
The new plan is only available to Walmart and Sam’s Club associates working in certain facilities in Arkansas. Our provider network includes Mercy providers in Joplin, Fort Smith, Rogers, Oklahoma City, Springfield, Joplin, Rolla, St. Louis and the surrounding areas, as well as select Mercy affiliated providers throughout Missouri, Arkansas, Oklahoma, and Illinois.
There are several differences between the plans. First, the Accountable Care Plan offers a single co-pay that covers in-network physicians office visits as well as most lab work or x-rays, when performed at the time of the office visit. Because you don’t have to meet the deductible for these services, there’s no HRA or HSA account.
Second, the Accountable Care Plan features a select network of local providers specifically aligned with Mercy to manage your care. While this narrow network is designed to improve the coordination and management of your care, there is no coverage for using out of network providers except in an emergency.
Third, the Accountable Care Plan offers the lowest premium cost per paycheck of all the plans offered in the area.

Mercy Accountable Care Plan represents a broad care network available to Walmart and Sam’s Club associates and their dependents. You’ll have access to the full range of specialists and subspecialists and the latest treatment options if a high level of care is needed. In addition, Mercy caregivers who are part of Mercy Clinic are linked by an electronic health record that’s available to them 24/7, so you can be sure they’ll always have your most up-to-date health information. In addition, if your physician is with Mercy Clinic you have access to parts of this record through MyMercy, our online portal that allows you to make appointments, contact your care team, get lab results, and more, whenever and wherever you want.

While not required, having primary care physician may improve your overall health and care coordination. If you need help locating a primary care physician, please visit our online provider directory. Your copay for an office visit with in-network primary care providers is $35. Eligible in-network preventive services are covered at 100%.
Your primary care physician knows your health history best and can recommend a specialist who is best suited to meet your needs. Please note that in order to receive coverage by your plan, the specialist you see must be part of Mercy Accountable Care Plan network.
The Accountable Care Plan offers no out-of-network coverage except in an emergency. If you see a local provider who isn’t in the network or seek non-emergency care while traveling outside the area, you won’t be covered.
To keep costs low for you and the plan, prescriptions are only covered at a Walmart or Sam’s Club pharmacy. Speciality drugs must be purchased from Walmart Specialty or Accredo Specialty Pharmacy (a subsidiary of Express Scripts). When purchasing prescriptions by mail order, participants may use either Walmart or Express Scripts mail order.
Finding a Provider
A primacy care provider is a physician or advanced practitioner who acts as your health care advocate and oversees your preventive and general care. He or she also helps coordinate care and services with specialists when needed. In short, a primary care provider helps you get healthy and stay healthy. Although choosing a primary care provider is not required, it is encouraged for these reasons. Your copay for an office visit with an in-network primary care provider is $35. Medical Records & Appointments
MyMercy provides you a summary of your health information based on your care team’s entries in Mercy’s electronic health record. You can view your test results, see your appointments (both future and in the recent past), treatments you’ve received such as immunizations, any allergies you may have, and what medications your records show that you are taking (both those your care team has prescribed and any that you have reported to your care team).

I forgot my User ID. What should I do?
Click the Forget Your User ID? link on the sign-in page. You will be asked for the name and e-mail address you used to establish your MyMercy account, and if you are able to provide this, your User ID will be sent to your designated e-mail.
I forgot my password. What should I do?
Click the Forget Your Password? link on the sign-in page to reset your password online. MyMercy will walk you through the steps for resetting your password.
How can I make, cancel or reschedule an appointment with my doctor?
After logging into your MyMercy account, point to the Appointments option in the navigation bar and click on Schedule an Appointment in the drop-down menu. You will be taken to a page that shows your Primary care doctor’s first available appointment, which you can select and then click Make Appointment. If you want to search for other dates and times, select the Advance Search tab. There you will be able to search for appointment dates and times that more closely meet your needs.
For Specialists, you will follow the same process, but will only be able to request an appointment.
Appointments previously made can be cancelled online with more than 24 hours notice. However, appointments that will occur in less than 24 hours cannot be canceled online. Please contact your care team’s office directly to cancel or reschedule these appointments.
How can I refill a prescription?
If you have refills remaining, just contact your pharmacy and request a refill.
How can I renew a prescription?
If you have no refills remaining, you can ask your care team to renew your prescription through MyMercy. Log into your MyMercy account, point to the Prescriptions option in the navigation bar, click on Renew Your Prescriptions option in the drop-down menu, and fill in the requested information.
What’s the difference between a renewal and a refill?
A prescription renewal is needed only when you have no more refills remaining on a prescription. A refill means that your care team has authorized you to receive a new supply of your drug for a given amount of time and a limited number of re-orders. If your care team has either told the pharmacy “no refills” or if your limited number of refills has expired, you can use MyMercy to request a renewal from your doctor.
